Prior to submitting an application for your child to attend Clear Horizons Academy, we encourage you to come tour our school. Simply call the school at 801-437-0490 to arrange a time when you can visit our facilities, observe our classes in action and ask questions.
Once an application has been completed and submitted, you will be contacted within two to four weeks to schedule an observation where you will visit the school with your child. This observation will help school administration to get to know your child and assess what the best class placement will be.
To be considered for enrollment, the following must be completed and returned to the school:
- Student Application & Information Form
- Copy of Most Recent Federal Tax Return
- Copy of Two Most Recent Paycheck Stubs
- Copy of Most Recent IEP (if available)
- $100.00 Application Fee

Summer Program: Clear Horizons Academy offers an optional summer program that focuses on the social/emotional development of children with all needs. While currently enrolled students are given priority, a child need not be enrolled in Clear Horizons Academy to participate in the summer program. The enrollment deadline for our 2014 program is May 9. See our Summer School registration form for more information.
